Background: Foot self-care is central to preventing diabetes-related ulcers and amputations, yet evidence on sex differences in Andean populations remains limited. Objective: To examine sex differences in foot self-care practices among adults with diabetes in Cañar, Ecuador, and determine whether any difference persisted after accounting for sociodemographic, clinical, knowledge, and attitude variables. Methods: A cross-sectional secondary analysis was conducted using complete data from 228 adults with diabetes (95 men and 133 women). Results: Participants had a mean age of 59.8 years (SD=10.4), and 58.3% were women. Women had a higher mean practice score than men. High practice was observed in 41.4% of women and 27.4% of men. After adjustment for age, diabetes duration, residence, primary caregiver, foot lesions, study site, knowledge, and attitudes, women had a 0.35-point higher practice score and a 1.56-fold higher prevalence of high practice. Conclusion: Women demonstrated modestly better foot self-care than men. Sex-responsive diabetes education, with strengthened behavioral support for men and site-specific implementation, may help reduce preventable foot complications in Cañar
